Labour health spokesperson Marie Sherlock TD has described the decision to strip four of the six HSE regions of control over day-to-day spending as a damning indictment of Government health policy, just 18 months after the new regional structures were introduced.
Deputy Sherlock said:
“This is an extraordinary reversal of one of the Government’s flagship health reforms. Just 18 months after regionalised budgeting was introduced, four of the six regions are effectively having control of their day-to-day spending taken away from them.
“The entire purpose of these reforms was to devolve decision-making and financial responsibility to regional level. Now, at the first serious test, Government is pulling financial control back to the centre. That is not reform. It is an admission that the system they introduced is not working as intended.
“Serious questions must be answered by the Minister for Health about how we got here. The HSE is facing a burgeoning deficit because Government has failed to provide realistic budgets that properly reflect demographic pressures and growing demand, while at the same time failing to maintain adequate financial control.
“Regional budgets were supposed to be built around a population-based resource allocation model, matching funding with the healthcare needs of the population. Yet it is increasingly clear that the funding being offered to regions is not adequately reflecting the level of demand they are actually facing.
“What makes this particularly alarming is what happens next. A spending clampdown on this scale will not exist on a spreadsheet. It will be felt in hospitals, clinics and community services. Patients and frontline staff cannot be made to carry the can for Government’s failure to properly fund and manage the health service.
“We are heading towards another extremely difficult winter for our health service. The last thing patients need is a panicked retrenchment in spending because Government failed to get its budgets right in the first place.
“It is also extraordinary that, this late into 2026, a number of our major hospitals are still operating without agreed service level agreements for this year.
“The Minister now needs to explain how a flagship reform has unravelled so quickly, what impact these new spending controls will have on services, and what protections are being put in place to ensure patients do not pay the price.”
